We welcome self-pay patients. If you do not have health insurance, please be prepared to make a payment at the time of service or you may speak with our office to discuss payment options.
Please bring your insurance card with you to your first visit as it needs to be verified and scanned into the patient’s chart as well as to subsequent visits. We will submit the claims directly to your insurance company. As each policy varies, you may be responsible for additional procedures or services determined by your insurance carrier.
Parents/Guardians must know their insurance and update the front office with any changes before seeing the doctor. You are responsible for obtaining any necessary referrals to your participating specialist, lab or radiology group before going to that appointment. Referrals require at least 5 days’ notice.
